Prestige Adult Foster Care is a Massachusetts state-licensed, CARF accredited and a community-based supportive living program that assists people over the age of 16 and adults with aging issues, medical conditions, mental illness, developmental or physical disabilities who cannot live safely at home.
What is Adult Foster Care?
Also referred to as Adult Family Care, is a federally funded MassHealth program that is designed to provide living alternatives to individuals who are elderly or disabled with performing activities of daily living (ADLs) and instrumental activities of daily living (IADLs). ADLs include activities such as eating, toileting, dressing, bathing, transferring, and walking. IADLs are activities related to independent living that are incidental to a member’s care, such as household-management, laundry, shopping, housekeeping, meal preparation transportation, and monitoring medication.
How Does AFC Work?
Prestige AFC provides and trains caregivers to deliver care in their homes. A Registered Nurse and Case Manager visits the home, monitors health needs, and develops a plan of care. A care manager provides case management through regular visits, ensuring the quality of care and access to resources.
