Members/Clients must:
- Be sixteen (16) years or older.
- Be eligible for MassHealth coverage.
- Have a medical or mental condition resulting in need for assistance with activities of daily living (ADLs) for at least one of the following: walking, eating, full body dressing, using the toilet, transfers (bed to chair).
- Be clinically approved by primary care physician (PCP) and MassHealth.
