Prestige Adult Foster Care is a Massachusetts state-licensed, CARF accredited and community-based program.
We provide specialized assistance to individuals aged of sixteen (16) years or older who are affected by a myriad of medical, physical, and or cognitive challenges.
What is Adult Foster Care (AFC)?
Adult Foster Care is a federally funded MassHealth program that is designed to provide living alternatives to individuals who are elderly or disabled. These individuals usually require assistance while performing activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
ADLs include activities such as eating, toileting, dressing, bathing, transferring, and walking. IADLs are activities related to independent living that are incidental to an individual’s care, such as household management, laundry, shopping, housekeeping, meal preparation, transportation, and medication monitoring.
How does an Adult Foster Care works?
As a licensed Adult Foster Care program we provide industry-specific training to caregivers which assist them in delivering care in their homes.
We work with a team of expert registered nurses and Case Managers who carry out monthly home visits, monitor members’/clients’ health needs, and develop personalized care plans.
